"The Office of Contracting and Procurement, on behalf of the District of Columbia Department of Health, seeks a vendor who can provide clinical telehealth services to students in school health suites. The contractor shall also provide clinical program support and case management. The objective of this procurement is to enhance student care through real-time virtual health consultations and scheduled telehealth appointments. The services will support approximately 187 public school health suites across the District and integrate with existing workflows managed by school nurses and health technicians. Services include acute consultations, scheduled follow-ups, case management for complex conditions, and nurse consultations for clinical guidance. "

Capacity Building Support Services

Chronic Disease Division (CDD) seeks a contractor to coordinate and implement 20 Living Well DC workshops through the Self-Management Resource Center (SMRC) for District residents with chronic diseases. The SMRC programs help people and their caregivers manage their symptoms, improve quality of life, and reduce healthcare costs. The SMRC program includes the following Living Well DC workshops; Chronic Disease Self¬ Management Program (CDSMP), Diabetes Self-Management Program (DSMP), Cancer Thriving and Surviving (CTS), and Tomando Control de su Salud. Implementing these Living Well DC workshops in community settings such as senior centers, residential facilities, and community-based organizations (CBOs) with an emphasis on locations with the highest needs in Wards 5, 7 and 8, will help CDD achieve its mission by providing District residents struggling with chronic disease with the necessary information and skills to manage their conditions.
